Suggest what to pilot with five people and what to ask them. Provide a final improved version of the survey and an analysis plan note for each question. Add a short list of biases explained in plain language with an example of each using invented questions, and a method for testing the survey with cognitive interviews, in which participants explain what they think each question means. Include a plan for checking the response data for straight-lining and other quality issues. Also add a quick check of how the survey will be analysed, so that every question has a purpose, and a rule for dropping questions that have no planned use.
