Asana Intake Form Design for Work Requests
Design a request form with fields that route work correctly.
The Open buttons send the prompt text as currently shown below to that site only when you click. Fill in the placeholders first.
[VERIFY] is an editorial flag, not a blank to fill. Wherever the prompt shows [VERIFY], check that item against a reliable source before you use it.
Design an Asana form for incoming requests to [TEAM_NAME]. Types of request we receive: [REQUEST_TYPES]. Information the team always has to chase: [CHASED_INFORMATION]. Who triages and how fast: [TRIAGE_PROCESS]. Systems that depend on the form: [DEPENDENCIES]. List each question in order with its field type (single line, long text, dropdown, date, number, file), whether it is required, the help text, and what it maps to in the task (name, description, custom field, assignee, section). Use conditional questions where one answer changes what we need next. Keep the form under ten questions and, for each one, ask whether the team would act differently depending on the answer; cut it if not. A form that takes more than three minutes will be avoided. Suggest rules that should run on submission, such as setting priority from the dropdown or notifying the triage owner, described in plain words and marked [VERIFY] where the rule builder options are uncertain. Write a confirmation message for the requester that sets the response time honestly, and offer an alternative for people who need to attach a long document. Add a test plan of three sample submissions, one deliberately incomplete, with what you expect to see. Do not ask for personal data we do not need.
