Project Post-Mortem Report
Capture lessons after a project ends.
The Open buttons send the prompt text as currently shown below to that site only when you click. Fill in the placeholders first.
Write a post-project review. Project, goals, and results: [PROJECT_AND_RESULTS]. Plan versus actual (schedule, budget, scope, quality): [PLAN_VS_ACTUAL]. Team observations: [OBSERVATIONS]. Produce a report with: summary; goals and outcomes table (goal, measure, target, actual); plan versus actual with variances computed (variance = actual - planned, percent of planned) and arithmetic; what went well and why; what did not and why (root causes, not blame); decisions that mattered; risks that materialized; stakeholder feedback; lessons for the organization (specific and actionable); recommendations with owners; and appendices. Add rules: separate facts from interpretation, include quotes from the team with permission, and share widely. Include a one-page executive summary (150 words) and a checklist for follow-up (are the lessons embedded in templates and checklists?). Use only the information provided and mark gaps in evidence. Limit the report to four pages. Express each lesson as 'When X happens, do Y, because Z' so it can be applied, and include a final table of lessons with the template, checklist, or process it should change.
