Design a discovery survey. Goal and decision it informs: [GOAL_AND_DECISION]. Audience and size: [AUDIENCE_AND_SIZE]. Distribution channel: [CHANNEL]. Time limit: [TIME_LIMIT].

Produce: a survey of 10 to 14 questions, with the question text, type, response options, and what each answers. Include a screener, behavior questions (frequency, last time), problem severity (rated with a clear scale), current solution and spending, open question on the biggest frustration, and optional follow-up consent. Avoid leading, double-barreled, and hypothetical purchase intent questions.

Add: an invitation (80 words), a sample-size note (a rough guide only; small surveys give directional, not precise results), an analysis plan (counts, cross-tabs, coding open responses), a bias list (self-selection, recall), and a data protection checklist for [JURISDICTION] [VERIFY]. Show before and after rewrites of three poor questions. Do not invent responses.

Provide a table of questions with the response scale and the decision each one informs, drop any question that informs no decision, and state the expected completion time computed at about 8 seconds per closed question and 45 seconds per open question.
