Interview me to create an outcome measurement plan for [PROGRAM_NAME]. Ask one question at a time and wait: who participates and how many; what changes we expect and when; what data we already collect; staff time available; participant burden and consent; and any funder requirements [FUNDER_REQUIREMENTS]. Then produce a plan as a table: Outcome, Indicator, Data source, Method (survey, attendance records, interview), Timing, Responsible, Target, and Baseline. Keep to no more than eight indicators. Add: survey questions (5 to 8, plain language, one idea each) with answer scales; a consent statement template; a data management note (storage, access, anonymization); a plan for analyzing results without statistical tools; and guidance on how to report negative or unclear results honestly. Explain the difference between outputs and outcomes in two sentences, and flag any indicator we cannot realistically collect. End with a pilot suggestion: test the survey with five participants. Prefer methods that fit into work staff already do, such as attendance sheets and short exit surveys, and warn me about any measure that could pressure participants or collect more personal data than we need.
