[VERIFY] is an editorial flag, not a blank to fill. Wherever the prompt shows [VERIFY], check that item against a reliable source before you use it.
Plan a survey email. Purpose and decision the results will inform: [PURPOSE]. Audience: [AUDIENCE]. Survey link and length: [SURVEY_DETAILS]. Incentive if any: [INCENTIVE]. Write the invitation in full, with two subject line options, an honest time estimate such as three minutes, who will read the answers and what will change as a result; a one-line reminder to non-responders after four days; and a thank-you page text. Draft eight neutral questions, none leading or asking for two things at once: two ratings on a five-point scale (such as how satisfied are you with X, and how likely are you to recommend us from 0 to 10), two choices, two multiple-select and two open questions. Ask only questions you will act on. Analysis: response rate is responses divided by invitations (90 of 600 is 15 percent); share satisfied is answers of 4 or 5 divided by all answers (63 of 90 is 70 percent). Explain how nonresponse can bias results, and compare responders with the list to spot it, for example only recent customers replied. Report counts as well as percentages. Say how answers are used, whether they are anonymous, and how long they are kept [VERIFY]. Plan to share what you heard and what you will do within a month, with a personal reply to anyone who wanted contact. Label interface details you cannot confirm [VERIFY].

Invented numbers. 600 invited, 90 responded (15 percent). Satisfied (4 or 5 on a five-point scale): 63 (70 percent). Would recommend (9 or 10 of 10): 31 (34.4 percent). Open comments: 28; themes: delivery time 9, price 6, product quality praise 8, other 5. Response bias: responders are likely more engaged than non-responders. Follow-up message: 'Thank you. You told us delivery time matters most; from June we show delivery dates at checkout.' Arithmetic: 90 / 600 is 15 percent response; 63 / 90 is 70 percent satisfied; 31 / 90 is 34.4 percent; the open comment themes add to 28.
