Contributing factors: no expiry alert, certificate owner unclear. Actions: add expiry monitoring, document ownership, add a test. Add a review checklist: the tone is blame-free, actions are specific and testable, and each action is a Jira issue [VERIFY the integration]. Add a 45-minute facilitation guide: 5 minutes ground rules, 15 on the timeline, 15 on contributing factors, 10 agreeing actions with owners and dates, with questions about systems such as what made the mistake easy and what would have made the problem visible earlier. Track actions weekly until closed and share a summary within a week. Keep customer data off the page and tag uncertain tool details [VERIFY].
