Rate usability problems. Observations from sessions (issue, participants affected, task, quotes): [OBSERVATIONS]. Number of participants: [N_PARTICIPANTS].

For each issue compute frequency (participants affected / total) and assign severity using defined levels: Critical (prevents task completion), Serious (causes significant delay or errors), Moderate (minor delay, recoverable), Minor (cosmetic or preference). Define impact on the user and the business. Combine into a priority score with a transparent rule (for example severity weight x frequency), shown with arithmetic.

Return a table: Issue | Evidence | Frequency | Severity | Priority | Cause hypothesis | Recommended fix | Effort guess. Group related issues, separate usability problems from feature requests, and note positive findings to preserve. Provide an executive summary (150 words) and a decision list for the team. Add a caution: small samples show problems but not their exact prevalence. Do not invent observations.

Report the issues in a table sorted by priority, add a one-sentence recommendation for each, and a count of issues by severity so the team can see the total picture. Recommend which five to fix before launch.
