Survey Ethics and Privacy Checklist

The Open buttons send the prompt text as currently shown below to that site only when you click. Fill in the placeholders first.
[VERIFY] is an editorial flag, not a blank to fill. Wherever the prompt shows [VERIFY], check that item against a reliable source before you use it.
Check a survey for ethics and privacy. Survey draft and purpose: [SURVEY_DRAFT]. Data collected (identifiers, demographics, sensitive questions): [DATA_COLLECTED]. Storage and tools: [STORAGE_AND_TOOLS]. Jurisdiction: [JURISDICTION]. Review: purpose and necessity of each question (remove what you will not use); informed consent text (who we are, why, how long, voluntary, how data is used, who sees it, retention, how to withdraw); anonymity versus confidentiality; sensitive data and special categories (needs extra care and often a legal basis [VERIFY]); children and vulnerable groups; incentives and fairness; data security (access, encryption, vendor terms); retention and deletion; sharing and reporting (small-cell suppression, aggregation); and respondent rights (access, deletion). Return a table: Item | Status | Risk | Fix. Provide a revised consent paragraph (100 words) and a plain-language privacy note. Recommend review by a privacy specialist for regulated contexts. Do not give legal conclusions. Produce a revised version of the survey's consent text and the list of questions to remove or make optional, with a reason for each, and a retention date by which the raw data will be deleted.
