Design a customer health score. Customer types: [CUSTOMER_TYPES]. Available signals: [SIGNALS]. Business goals (retention, expansion, satisfaction): [GOALS]. Team size and tools: [TEAM_AND_TOOLS]. Return: 1. Score design: five categories (adoption, engagement, outcomes, relationship, commercial) with the signals in each, how each is measured, and the weight. Show a table and make the weights add to 100. 2. Scoring rules: how raw values convert to 0 to 100 (thresholds or percentiles by segment), with an example: logins per week: 0 = 0 points, 1 to 2 = 40, 3 to 5 = 70, 6 or more = 100. 3. Status bands: green, amber, red with definitions and the action expected for each. 4. Example calculation for two accounts with the arithmetic. 5. Reliability checks: does score correlate with renewal in past data? Bias checks across segments; how to handle missing data. 6. Operating rhythm: weekly review of reds, monthly trend, quarterly recalibration. 7. Pitfalls: too many signals, scores nobody understands, gaming metrics, and ignoring qualitative information (the account manager's note). Provide a spreadsheet layout. Mark assumptions clearly and do not use personal data beyond what is needed.

Weights (total 100): adoption 30, engagement 20, outcomes 20, relationship 15, commercial 15. Adoption: weekly active users / licensed users. Rule: below 30 percent = 0 to 9; 30 to 59 = 10 to 20; 60 or more = 21 to 30. Account Pine Dental: adoption 72 percent = 27; engagement (monthly logins 14 against norm 12) = 20; outcomes (goal met, no-shows down 5 points) = 16; relationship (champion plus finance contact) = 12; commercial (paid on time, 12 months left) = 15. Total = 27 + 20 + 16 + 12 + 15 = 90, green. Account Reed Logistics: adoption 22 percent = 6; engagement 8; outcomes 5; relationship 4 (champion left); commercial 9. Total 32, red. Bands: green 70 or more; amber 50 to 69; red under 50. Red triggers a call within 2 days. Check against past renewals before trusting the score.
