Add instructions to run it as an anonymous survey plus a facilitated discussion, a scoring sheet with area averages, thresholds for green, amber, and red (adjustable), and a recovery plan template for red areas (cause, action, owner, date). Include a way to compare perceptions across roles (sponsor versus team) and highlight gaps of 2 or more points. Provide a summary format for the sponsor (one page). Use only my project details. Write the 24 questions as complete sentences answerable on the 1 to 5 scale, with anchor text for 1, 3, and 5 for at least six of them as examples. Provide the scoring sheet and the thresholds for each colour band, and say how to handle missing answers.
