Facilitate a definition of done workshop. Team and product type: [TEAM_AND_PRODUCT]. Quality problems we have had: [QUALITY_PROBLEMS]. Tools and environments: [TOOLS]. Produce: a 60-minute agenda; an exercise to list what usually gets skipped; a draft definition of done with checklist items at three levels (story, sprint, release): code reviewed, tests written and passing, accessibility checked, documentation updated, security checks, deployed to a test environment, product owner acceptance, monitoring in place. For each item: who checks it and how it is evidenced. Add a difference table: acceptance criteria (per story, specific) versus definition of done (all stories, general). Include a plan to tighten the definition gradually, a rule that unfinished items return to the backlog, and a retrospective check after three sprints. Provide a poster-style summary of ten lines. Use my details and avoid dictating tools I have not named. Produce the definition as a numbered checklist of no more than twelve items, each phrased as a check that has a yes or no answer, and mark which items are automated and which need a human to confirm.

Story level: code reviewed by one other person; unit tests written and passing; acceptance criteria demonstrated; accessibility check (keyboard and labels); documentation updated; deployed to test environment. Sprint level: no open severity-one bugs; product owner accepted; release notes drafted. Release level: security scan clean; monitoring and alerts in place; rollback tested. Automated: tests, scan, deployment. Human: review, acceptance, accessibility. Difference: acceptance criteria are specific to a story; the definition of done applies to all. Unfinished items return to the backlog. Tighten gradually: add performance check in sprint 6; review after three sprints. Poster: ten lines, one per check.
