Compute coverage: percent of requirements with at least one test (count with test / total, shown). Add an update routine, change control for requirements, and a summary for stakeholders (100 words) explaining coverage and gaps. Note when a matrix is overkill (small projects) and when it is vital (regulated products). List orphaned requirements and orphaned tests separately with a count of each, and compute coverage as requirements with at least one passing test divided by total requirements, giving the percentage. Highlight the three requirements with the highest risk and the weakest test coverage first, and recommend whether each needs an extra test, a reworded requirement, or removal from the release.
