Design a fillable form in Word. Purpose: [PURPOSE]. Who fills it in and who reads it: [USERS]. Information needed: [FIELDS]. Rules (required answers, allowed values, signatures): [RULES].

Return a layout plan: sections, field labels, plain-language instructions, and for each field the control type (plain text, rich text, drop-down list, combo box, date picker, check box) with list entries or date format. Explain how to build it with content controls on the Developer tab and protect the document so only the fields can be edited [VERIFY menu names]. Limits of Word forms (no automatic validation of every kind) and when a web form would be better. Provide an instruction block for the top of the form and a thank-you or next-steps message. Privacy: collect only what you need, tell people why, store forms securely, say how completed forms are collected, and avoid copies kept in many places. Test script with three users: a careful one, a hurried one and one using a keyboard only, noting what to fix after each.
