Severity ratings use a stated scale and consider frequency, impact, and persistence. 5. Distinction between usability problems, preferences, and feature requests. 6. Quotes and counts supporting each finding (for example 4 of 6 participants failed task 2). 7. Recommendations specific, feasible, and linked to a problem. 8. Positive findings noted so they are protected. 9. Limits stated honestly. Return a scorecard, a re-ranked list of findings with a recommended severity and why, any findings that should be downgraded or dropped, and missing follow-up tests. Add a one-page summary for stakeholders with the top five issues and the decision needed. Don't add findings that are not in the report.
