Form Accessibility Audit: Labels, Errors and Keyboard Use
Check a web form for common barriers.
The Open buttons send the prompt text as currently shown below to that site only when you click. Fill in the placeholders first.
[VERIFY] is an editorial flag, not a blank to fill. Wherever the prompt shows [VERIFY], check that item against a reliable source before you use it.
Audit a web form for accessibility. The form fields, labels, help text, error messages and submit behaviour as I can describe or paste: [FORM_DESCRIPTION]. The audience: [AUDIENCE]. The platform used to build it: [FORM_PLATFORM]. Check for visible labels tied to inputs, grouped options with a legend, required fields marked in text as well as colour, helpful input hints, error messages that identify the field and how to fix it, errors shown near the field and summarised, no reliance on placeholder text as the only label, keyboard use with a logical order and visible focus, time limits, autofill support, and a confirmation page. Create a table with Field or Area, Finding, Impact, and Fix, and rewrite three error messages I provide. Suggest a quick test: complete the form using only the keyboard, zoom to 200 percent, and use a screen reader for one path. Mark platform-specific advice as [VERIFY], and do not claim the form meets any standard.
